Sharing the field or practical experience in a case report form

  1. Obtain: – I got a call from owner from far village near river bank 45 km from my hospital early in the morning that his cow is in labor pain since last night and cant able to give birth.
  2. Discuss:- first I ask some the question from owner about the situation of calf as well as cow. Discuss the situation with my senior doctors. What to do to give relief from pain to animal?
  3. Build:– Built my team which have an attendant and as assistant veterinary field officer which have little knowledge about drug but not know about surgical techniques.
  4. Gather: – With lots of obstacle in path we finally reach to the place with our team and get previous history about the cow. Gather the clinical information by talking temperature, pulse, respiration and by placing hand on uterus get information of position of calf, presentation of calf, posture of calf, condition of mother, condition of calf (live or dead), whether twin birth etc.
  5. Perform:– By watching all the clinical condition it’s time to decide whether go for manual traction or for caesarian section and what owner wants from us as well as what literature said for such condition but the priority to save the life of cow.
  6. Condition: – Calf is alive: 1st we try to correct the position of calf inside the womb so that it pulled out easily by traction. If the 1st case not occur due to some complication we go for C-section for safeguard of life of calf. Inverted ‘L’ block performed 2.5 inch below the lumber vertebrae and behind the last rib in paralumber fossa and site was prepared for operation. The incision is made vertically in the middle of the paralumbar fossa, starting approximately 10 cm ventral to the transverse processes of the lumbar vertebrae and continuing ventrally, far enough to allow removal of the calf. Closure of the abdominal wall is straightforward and relatively easy. Give fine incision at the site 1st the skin than all muscle layers. Find the uterus and give fine straight incision on uterus enough to remove the calf. The calf and fluid are removed carefully. Clean the uterus with normal saline and close the uterus first by giving lambart suture with catgut no.3. Followed by suture all the muscle layer with continuous suture using catgut no.3. Finally suturing were made for skin with cotton or silk. Antiseptic precaution were made.  Clean the calf nostril, eye and give mouth to mouth breath.
